    The first box is for your zip code, and the second box is for your state/city/street information. However, the input process is a bit different from simply typing in the information. Instead, please kindly click on the 'Search' button next to the first box.

    This will bring up a new pop-up window. In this new window, please enter your zip code and click on the 'Search' button. It will then populate a list of address search results at the bottom which you can pick from. Please double click on the correct address from the list. The new pop-up window will then close automatically, and you will see the original window, with the first and second boxes populated.
